If you think you might be eligible and want to apply, complete an Access Request Form and return it to NDIS

Get your evidence

When you apply, you must also send evidence to prove your age, Australian citizen or permanent resident, and your disability.

NDIA Decision

Once you have provided everything, NDIS will decide if you are eligible or ask for more information if need be.
